| Insurance & Billing |
We will file your insurance and attempt to
verify coverage prior to treatment. Verification of benefits is
not always a guarantee that your insurance company will pay for
physical therapy services and any portion that insurance does
not cover is ultimately the patient’s responsibility.
Premier Physical Therapy will collect all co-pays, co-insurance
and deductibles upfront at the time services are rendered. If
you are unable to make the required payments alternate payment
plans are available upon request.

and we will verify your insurance coverage and your benefits for
